Refund Policy – Peak Performance Sport & Spine

At Peak Performance Sport & Spine, we are committed to transparency and fairness in our financial policies. Please review our refund policy below:

1. Services Rendered

All services that have already been performed are non-refundable, regardless of the payment method used. This includes, but is not limited to:

  • Chiropractic adjustments
  • Physical therapy sessions
  • Massage therapy
  • Other manual or rehabilitative therapies

Once a service is provided, the associated cost is considered earned and is not subject to return or refund.


2. Durable Medical Equipment (DME)

All purchases of DME (e.g., braces, supports, orthotics) are final and non-refundable. For sanitary and liability reasons, we do not accept returns or exchanges on any medical equipment, whether used or unused.


3. Prepaid Service Packages

Patients who purchase discounted service packages and do not use all sessions may request a pro-rated refund under the following conditions:

  • Refunds will be calculated based on the single service self-pay rate per service (not the discounted rate).
  • The full cash value of all services already used will be subtracted from the amount paid.
  • The remaining balance (if any) will be refunded to the original form of payment or via clinic check.

Example Calculation:

Description Amount
Package Paid (12 visits) $450
Original Cash Price per Visit $50
Number of Visits Used 4
Value of Services Rendered $50 × 4 = $200
Refund Due $450 – $200 = $250

Note: If the value of services already rendered exceeds the original payment amount due to pricing adjustments or bonus inclusions, no refund will be issued.


4. Refund Request Procedure

  • All refund requests must be submitted in writing to the front desk or by email at reception@peakperfomss.com.
  • Please allow up to 14 business days for processing.
  • Refunds will not be granted for requests made more than 6 months after the original purchase date unless otherwise required by law.

5. Special Considerations

We reserve the right to make case-by-case exceptions for circumstances involving medical hardship or discontinuation of care upon doctor’s recommendation.
